New Patient Resources & Paperwork

Congratulations on taking your next step to better health!

Please complete all of the paperwork below before coming to your new patient appointment. Taking the time to read these resources carefully and thoroughly complete the paperwork will allow us to utilize your time at your new patient appointment most effectively. We do know that we’re asking for your time before this relationship even begins, but unlike traditional medicine, in functional medicine the ownership of health belongs to the patient. We’ll guide you and teach you what is best for your body to help you achieve your unique health goals but we can’t do it for you and that decision to work for your health starts now! These resources and questionnaires have been carefully selected to help us (and you!) know your body in depth before we begin making treatment recommendations. When we know how your body works we can identify how to make it healthy much faster which saves you time and money!

New Patient Paperwork- Please print and complete your health history information and bring it with you to your new patient appointment.
HIPAA & Practice Authorization- This is your standard office paperwork. Please read through it carefully and sign and initial all appropriate places. If you prefer not to print this out, please arrive to your appointment 15 minutes early to complete these documents. These must be completed prior to your new patient visit. Please make sure to read the New Patient Appointment Deposit Policy completely to ensure that you know the terms of your New Patient Appointment Deposit.
Timeline- This is a unique assignment that we ask our patients to complete. Please read the following information about the timeline and complete the timeline. If you have additional questions please feel free to contact us at [javascript protected email address] or watch our timeline video.
Sleep Questionnaire- Sleep is critical to health! Please complete this quick questionnaire so that we can help assess the quality of your sleep.
Readiness for Change Questionnaire- Lifestyle change has many components. Help us know where you are by taking the Readiness for Change Questionnaire. We want to tailor your program to your lifestyle and this gives us another layer of information that it would otherwise take us months to discover in the course of appointments! Let’s save your time and money and learn it up front!
